I came across this handwritten document in a field case that I get from a friend, and it left me to ponder: what is it?

I will give you a picture of this celebrities hair and forehead..and you get to have fun trying to figure out who it is!  Sounds fun to me.  I will have the full picture tomorrow afternoon- So guess on!  Oh, and here is a hint: She is famous!
